Grade 1 fatty liver: what it really means

"Grade 1 fatty liver" appears on a huge number of ultrasound reports. It's a useful warning, but not a diagnosis of liver damage.

What the grade means

The ultrasound grade describes how bright the liver looks, which reflects fat. It does not tell us whether there's scarring (fibrosis), which is what actually matters for the future.

What we check

  • Blood tests, including a simple FIB-4 score
  • Sometimes a FibroScan (elastography)
  • Diabetes, blood pressure, cholesterol and waist size
  • Alcohol use and medicines

What helps

  • Losing 5–10% of body weight, gradually
  • 150 minutes of activity a week, plus some strength work
  • Fewer sugary drinks and refined carbs
  • Little or no alcohol
  • Good diabetes control

Thin people get fatty liver too, especially with a larger waist. "Liver detox" products don't help and some harm the liver.
